Sir,
I beg most respectfully to state that I am a student of class 4 in your school. I would like inform you that I can not attend school from 07-09-2022 to 09-09-2022 due to my fever and cold. I have attached a medical certificate to this application.

May I therefore, pray and hope that you would  kindly enough to grant my leave of absence for those days .
